It is now easier then ever to keep informed of your benefits. From the links that you will find throughout this page you can download our health insurance Summary Plan Description (benefits book), find PPO providers in your area, dentists, get prescriptions online, hospitals and much more. You can also sign up for our Retirement Plan if you are eligible, and manage it if you are a participant.

We offer an excellent health plan that includes medical, dental, vision, pharmacy and life insurance. Our waiting period for enrollment is 90 days. Download an application from the link below, fill it out and send it or bring it to the office. Your application will be processed when you become eligible.

Retirement Plan:

After one year of service, you can be a participant in the Arok Team sponsored 401-K retirement plan. For more information call the office or click on the My 401-K link below.

Access your account if you are a participant of the Arok Team Retirement Plan. If this is your first time using the system, you need to create a user name and a password. Some basic information is required, if you don't have that information call the office.

>Redemption Fees Information. A redemption fee is assessed to a participant's account based on the number of days between a "round trip" purchase into and sale out of a specific fund, as determined by the mutual fund company. If fund shares are purchased and sold within a period of days that is shorter than the fund company's defined "holding period", a redemption fee may be assessed. Redemption fees are charged by the mutual fund and paid out of the participant's account.
